The show's third season explored the sisterly drama between Kim and Kourtney, and it appears the dynamic is no better between the two a season on.
The season four trailer begins with Kim and Kourtney sitting down together to be interviewed, with a producer off-screen saying: "Do you think people are going to be surprised to see the two of you together?"
Kourtney then admits the last season was "really hard," and that there's "been a lot of tension".
"It's really hard to watch people talking about you. I don't feel understood," she adds.
But towards the end of the trailer, tensions appear to rise between the sisters as they talk on the phone, with Kourtney telling Kim: "You're just a witch, and I hate you."
All the drama also seems to have impacted Kris, who's seen wiping away tears as she says: "I'm not even talking about it anymore.
"Family is the most important thing in my life, and it's my responsibility to make sure that I keep my family together."
Elsewhere in the trailer, Khloe's ex Tristan Thompson is still around — and the family has mixed feelings about him. While Kris praises Tristan's skills as a dad, Kourtney tells him: "I don't think you deserve Khloé."
On a positive note, Kylie — who was recently spotted with Wonka star Timothée Chalamet — seems to be having a great time, telling her sisters: "I feel like myself again. I feel amazing."
Meanwhile, Kourtney is currently recovering after undergoing fetal surgery last week.
In a recent Instagram post, Kourtney, who's expecting her fourth child and her first with husband Travis Barker, thanked doctors for saving the life of her unborn baby and Travis for rushing home from Blink-182's European tour to be by her side.
The Kardashians seasons 1-3 are available to stream now on Disney+. Season 4 premieres on September 28.
